Steps to complete your China Visa Health Examinations
Step 1:
Download the Taiwan Visa Health Examinations form here: Health-Certificate-for-Residence-Application
Step 2:
Call for appointment with Health Screening Centre at +603-2037 2368 or email to wellness@tungshin.com.my
Step 3:
Bring along the Taiwan Visa Health Examinations form with passport size photo (White background) and an I/C or passport during your visit.
Step 4:
Complete your Health Examinations test.

Step 5:
You will receive a call from us once the report is ready for collection.
Prior Examination:
- No need for Fasting before examination.
- Bring along the Measles/ Rubella Vaccination card if available.
